In order to ensure a smooth passage through the security checkpoints being established within our university, we kindly request that you enter the following ID Card information into the system by no later than 15:00 PM on Friday, September 8th, 2023.

In case of an emergency, this information will be included on the ID Card as apoint of reference for healthcare members. Therefore, if you are unsure or lack the necessary details, it is advisable to leave the space blank.

The accurate and complete recording of this information in the system is of great importance to ensure security and seamless transitions.

Once we receive the complete information, the ID cards will be prepared, introduced into the system, and then distributed.

The ID cards currently held by academic staff with the old cards will become invalid. Therefore, all academic staff members are required to fill out this form.
